This position interacts with the current and potential residents, families
, and visitors. This position will be responsible for census development, community relations and community education with both existing and new referral sources for the facility. This position will receive and review admission information for the facility from various referral sources. Attention to detail, critical thinking, responding in a timely and professional manner and teamwork are essential to this role. This position will assess, have a good knowledge of, and sensitivity to, the needs, services and care coordination of, and for, their clients, residents or patients. They clarify a range of legal documents for residents and families as well. As a member of the interdisciplinary care planning team, this position identifies the psychosocial strengths and needs of each resident will assist with admission paperwork including social services assessments.


Must recognize and understand the psychosocial and emotional needs of 
residents. Position requires strong skills in communication, organization, management, and leadership. Must be able to perform essential functions of the position with or without reasonable accommodation. Must qualify for employment after a criminal background study, per guidelines of the Minnesota Department of Human Services.
